The injury occurred on March 16, 2024, and involved a malfunctioning hose connected to a vacuum blender used in the processing of poultry products.
According to reports from an Ontario court bulletin, the hose was responsible for carrying carbon dioxide, which is used to cool products during processing. During the incident, the hose ruptured, leading to the uncontrolled release of over 16,000 pounds of carbon dioxide into the deli area of the plant. A worker who was present in the vicinity at the time was critically injured due to exposure to the released carbon dioxide, prompting an immediate investigation into the circumstances surrounding the incident.
The investigation revealed alarming details regarding the safety measures in place at Maple Lodge Farms. Specifically, it was found that there was a complete lack of carbon dioxide sensors or alarm systems within the plant, which would have alerted workers to the dangerous situation.
